The challenge of mitral Valve-in-Valve in hostile territory
Transcatheter mitral valve replacement (TMVR) "valve-in-valve" in patients with a history of complex cardiac surgery represents a major technical challenge, particularly when septal anatomy is compromised. This study reports the case of a 55-year-old female patient, initially operated on for an incomplete atrioventricular canal with placement of a Carpentier PERIMOUNT 27 bioprosthesis, presenting with severe prosthetic degeneration associated with a heavily calcified interatrial septum.
The objective of this intervention was to perform a valve-in-valve implantation despite the impossibility of using a conventional transseptal puncture due to pathological septal rigidity. The clinical hypothesis tested here was based on the feasibility of a septal crossing strategy assisted by a Versacross system, followed by the creation of an arteriovenous loop to stabilize the placement of a SAPIEN 3 Ultra 26 prosthesis. This report documents not only the resolution of this extreme anatomical constraint but also confirms the functional durability of the valve at one-year follow-up, offering a concrete alternative to iterative surgical options in high-risk patients.
Méthodologie et prise en charge clinique
This case report describes a mitral valve-in-valve procedure in a 55-year-old female patient with a history of surgically repaired incomplete atrioventricular septal defect, with implantation of a Carpentier PERIMOUNT 27 bioprosthesis (Edwards, USA).
Le protocole d'intervention a dû pallier une difficulté majeure : l'impossibilité de réaliser une ponction transseptale conventionnelle en raison d'un patch interauriculaire sévèrement calcifié. La procédure a suivi les étapes suivantes :
- Access and crossing: Use of the Versacross system (Boston Scientific) to successfully perform the transseptal puncture through the calcified patch.
- Navigation: Placement of a deflectable catheter to advance a high-performance guidewire, captured in the left ventricle to establish a veno-arterial loop.
- Preparation: Septostomy performed using a 16 mm × 45 mm Atlas Gold balloon catheter (BD, USA).
- Implantation: Introduction of a 26-Fr DRYSEAL sheath (Gore, USA) into the left atrium, followed by the implantation of a SAPIEN 3 Ultra 26 bioprosthesis (Edwards, USA) with nominal inflation +2 cm³.
The technical success assessment was based on intraoperative imaging and clinical follow-up including a 1-year evaluation, confirming the long-term stability of the valvular function.
Gestion d'une bioprothèse mitrale dégénérée sur septum calcifié : une approche complexe
Le cas rapporté concerne une patiente de 55 ans, porteuse d'une bioprothèse mitrale (Carpentier PERIMOUNT 27) suite à une correction chirurgicale d'un canal atrioventriculaire incomplet. Bien que ce rapport démontre la faisabilité technique du valve-in-valve mitral en terrain hostile, il s'agit d'une observation unique (N=1). Cette approche souligne la nécessité d'une planification rigoureuse en cas d'antécédents chirurgicaux complexes, où les calcifications massives transforment une procédure standard en un défi technique majeur. La maîtrise des outils de franchissement septal est ici déterminante pour éviter les complications iatrogènes, comme observé lors de la tentative initiale de ponction transseptale.

- Résultats à long terme : La technique valve-in-valve transseptale constitue une option thérapeutique viable et durable, même chez des patients avec des antécédents cardiaques complexes, à condition d'une préparation minutieuse du septum.
